Sculpting without adding volume
The patient in this video is fit, lean, and younger. His face is already structured, but the midface and neck have started to lose tightness. Adding more filler would give him volume he does not need. A neck lift would give him downtime he cannot afford. Threads give him lift and definition without either.
Dr. Emer placed barbed threads along the midface, jawline and neck to pull tissue upward and inward, creating the sculpted look that comes from structure, not bulk. The lift is visible at the end of the session, and the result tightens further over the following six weeks as collagen forms around the threads.
What threads actually do
These are thick, barbed threads made of the same material used in dissolvable sutures. The barbs, shaped like small W patterns, run in multiple directions along the thread and grip the tissue as the thread is placed. That grip lifts the skin and fascia immediately, and the material itself stimulates collagen production as it absorbs over six to twelve months.
Dr. Emer customizes the placement for each face. For this patient, he used a tri vector approach: lifting the cheek, the fold, and the marionette line in one pass, then securing the threads at the back of the jawline to create definition without adding filler to the jaw or neck.
Why threads have controversy
Threads have a reputation for not working or for creating visible lumps under the skin. That reputation comes from poor placement. Older thread designs had weaker barbs that would slip if a patient smiled or laughed too hard, leaving visible curls under the skin. Modern threads have stronger, multidirectional barbs that hold tissue securely when placed correctly.
Dr. Emer has been using threads for years, and his patients who receive them consistently look tighter and younger than their peers who do not. His fiance has threads in his neck. The technique works when the provider understands facial anatomy and customizes the approach rather than following a one size fits all protocol taught by a company course.
Downtime and results
This patient had nerve blocks and numbing, so the procedure was comfortable. He will have minor swelling for a couple of days, then he can return to working out without restriction. The lift is visible immediately, and the tightening continues for six weeks as the threads stimulate collagen and the tissue settles into its new position.
This is not a replacement for surgery. It is a tool for younger patients who want to slow aging and maintain definition without the volume that comes from filler alone or the recovery that comes from a neck lift.
