Why silicone instead of hyaluronic acid
Dr. Emer used to perform glans enlargement with hyaluronic acid and PMA. After years of doing it, he found that the tissue of the head behaves like a sponge: it fills with blood easily, and it does the same thing with filler. That means hyaluronic acid can go down quickly, and the area tends to get bumpy.
Silicone does not behave that way. It builds thickness more reliably and integrates into the tissue over time. The trade is that it is permanent, so the technique and placement have to be right from the start.
What happens during the session
The patient is numbed with nerve blocks and topical cream. Dr. Emer injects microdroplet silicone into the dermis of the glans, building volume across the entire head, not just the corona. The area is very vascular, so pressure and technique matter.
A small amount of filler may spurt out during injection. That is not waste: it confirms the material is being placed in the correct layer. Most people will have some asymmetry or small lumps at the beginning, and those are addressed by continuing to fill and blend the area over multiple sessions.
Multiple sessions are part of the plan
This is not a one-and-done treatment. Dr. Emer limits the amount placed per session and builds the result over time. That approach reduces the risk of unevenness and allows the tissue to integrate the material properly.
Because silicone is permanent, conservative layering is the only way to control the outcome. If something goes wrong, correction requires surgery or localized steroid injection to shrink a specific spot.
Recovery and what not to do
Bruising is normal. No sex or masturbation for one to two weeks while the bruising resolves. Once the bruising is gone, normal activity can resume, but patients are told not to go overboard in the first couple of weeks.
Dr. Emer has had patients move filler to the wrong side by having sex too soon after treatment. The material needs time to settle into place before the area is subjected to friction or pressure.
After the session, patients are placed on daily Cialis and PT-141 peptide to increase circulation to the area. A small amount of nitro paste is also applied to help with blood flow.
What the result looks like
The head is visibly larger immediately after the session, and the result continues to firm and integrate over the following weeks. The corona is always made bigger, and the entire glans is filled to create proportion with the shaft.
There is a limit to how much volume the tissue will accept. At a certain point, the area will harden or thicken and will not take more material. The final size depends on the anatomy you started with, not just the amount of filler placed.
