What gynecomastia actually is
Gynecomastia is the enlargement of male chest gland tissue. It can be genetic, hormonal, or environmental, and it creates puffiness that stretches the nipple and areola. The result is a chest that reads as soft rather than flat, no matter how much the patient trains.
The patient in this video has hypertrophied glandular tissue sitting under the nipple. That tissue is what Dr. Emer removes to give the chest a flat, athletic, masculine look.
Why the gland removal is only part of the plan
Removing the gland alone leaves the skin stretched and the nipple positioned too low or too wide. Dr. Emer corrects both in the same session: he removes the excess gland, then tightens the areola and repositions it to sit at the lateral border of the pectoral muscle, where it reads as athletic.
He leaves a thin disk of tissue under the nipple to maintain blood flow and sensation. That layer prevents the nipple from looking scalloped or hollowed, and it keeps the chest from reading as over-corrected.
The lift that most practices skip
The areola lift is what separates a flat chest from a defined one. Without it, the chest can still indent when the patient flexes or leans forward, and the nipple hangs slightly even after the gland is gone.
Dr. Emer uses a technique adapted from what was once called a donut lift: a small incision around the nipple removes the stretched, pigmented skin and pulls the tissue tight against the chest. The scar sits at the edge of the areola, where it is nearly invisible once healed.
The complete MAXORIS™ chest plan
This is not just gland removal. The full plan includes liposuction of the chest and sides, fat grafting or filler to add muscle tone where needed, upper abdominal sculpting, and the nipple lift. Each step is designed to work with the others, so the chest finishes as one shape rather than a collection of separate corrections.
Candidacy and the exact combination of steps are decided in consultation, based on the tissue, the degree of gynecomastia, and the athletic look the patient is after.
