Sculpting, not reduction

The patient in this video is muscular and consistent in the gym. He has not had surgery before, and he is not looking to lose weight. He has small problem areas of stubborn fat and wants tighter skin. This is all about shaping and defining: giving him that six-pack, creating the shape at the edge of the border of his chest, and defining out his arms.

Dr. Emer uses fat removed from the flanks and back to build out the shoulders, upper chest, and buttock. The goal is the MAXORIS physique: broad, sculpted, and defined.

How Vaser liposuction works in this case

The session begins with tumescent anesthesia, a fluid injected into the areas where liposuction will be performed. The fluid hydrates the fat so it can be removed more easily. Then Vaser ultrasound is applied. Dr. Emer explains that the ultrasound pushes the fat cells away and aside safely so the fat can be harvested and used for the shoulders and buttock.

Vaser is used on the flanks, back, arms, and chest. On the arm, it allows Dr. Emer to get the cut between the bicep and the tricep and create the adhesion between the deltoid and the lateral part of the deltoid and the tricep. On the chest, it sculpts the chest wall to be very defined, giving a more masculine, less rounded, more squared off look.

Internal heating for tighter skin

After the fat is removed, Dr. Emer uses internal plasma radio frequency heating, which he calls Renuvion. This is internal heating that contracts and tightens the skin from the inside. In the video, he notes that you can actually smell it contracting the tissue. The goal is an even better look, a tighter look, and a longer lasting result.

On the lower abs, the internal heating is used to ensure no fat ever comes back and the area stays super flat and tight. Dr. Emer says it is snatching underneath the skin and tightening the skin.

Fat grafting to shoulders, chest, and buttock

The fat removed during liposuction is irrigated to get rid of excess fluid and impurities, then loaded into syringes for injection. Because the patient is very lean, there is only enough fat to do a little bit in the shoulders, but the goal is to cap out the shoulder and get a nice rounded look.

Fat is injected around the muscle in the shoulder to give a more broad look. On the chest, the goal is to create a little bit of lift and a little bit of upper chest. Dr. Emer says the patient wants some cleavage, so the goal is to give him some cleavage.

Gynecomastia correction

The session also includes gynecomastia surgery. Dr. Emer makes an incision around the current nipple and takes out a little bit of skin to tighten up the chest. He removes the intervening skin while leaving the deeper layers of the epidermis behind so the nipple retains its blood supply.

He tightens up the dermis and brings things closer together before opening below the nipple to take out any glands.
