When volume loss is medical, not cosmetic

The patient in this video has HIV-related lipo atrophy: deep volume loss in the face that changes structure, not just surface. Synthetic fillers would be expensive, temporary, and would not address the underlying tissue loss. Dr. Emer chose a different approach: cadaveric fat (Alloclae) placed throughout the face to rebuild the foundation and stimulate the patient's own fat production.

This is pan facial volumization, meaning the entire face is treated as one plan: midface, under eyes, temples, nasal labial folds, prejowl, jawline, and brow. The material is placed on bone and in tissue, and it integrates over weeks as swelling resolves and the body responds.

Why cadaveric fat works differently in HIV patients

Dr. Emer has treated HIV-positive patients for nearly 20 years, and the outcomes with fat grafting and cadaveric fat are consistently strong in this population. The material survives well, stimulates collagen production, and becomes permanent as the body builds its own tissue around it.

Sculptra, which works for many patients, did not function as well for this patient in the past. Alloclae gave him the volume and structure he needed in one session, with the option to refine later using micro droplet silicone or additional filler on the bone.

What the session looked like

The video shows the full procedure: Dr. Emer works through each area with a larger cannula, placing the material on bone and in tissue, building the jawline posteriorly to lift the face anteriorly, and blending the temple and brow to eliminate step-offs.

The patient left swollen, which is expected: the numbing medicine and the volume from the material itself create an overdone look on day one. Dr. Emer tells every patient that the main swelling resolves within one to three days, and the final result is visible at two weeks as the material settles and integrates.

Who this treatment fits

This is not a cosmetic filler session. It is a medical volumization plan for patients with HIV-related lipo atrophy or other conditions that cause deep facial volume loss. Candidacy depends on your medical history, the areas that need rebuilding, and whether your tissue will respond to cadaveric fat, which is exactly what a consultation covers.

Dr. Emer's practice is in West Hollywood and New York City. Consultations begin with photos submitted in advance so he can assess your case before you arrive.
