What massive weight loss leaves behind

This patient lost a significant amount of weight through diet, exercise and peptide therapy. The weight came off, but the skin did not. Loose skin covered his chest, stomach, arms and legs. The shape he worked for was hidden under tissue that no amount of training could tighten.

Dr. Emer and Dr. Roos planned a three phase surgical transformation: phase one addressed the torso with a full circumferential body lift, phase two lifted the chest and arms while contouring the calves and shoulders, and phase three completed the legs with a thigh lift and additional fat grafting to the chest and buttocks.

Phase one: the circumferential body lift

The first surgery removed skin all the way around the body. Dr. Emer used vaser ultrasonic liposuction to break apart fat cells and harvest fat for later grafting, then applied Renuvion plasma to contract the skin before the lift. The goal was to undermine the tissue so the body lift could close without excessive tension.

Excess skin was removed from the upper back, lower back, flanks and abdomen. The incisions followed the curve of the upper buttock and lower back, closed in three layers with drains placed to collect fluid over the following days. Harvested fat was grafted to the upper buttock to create shape and to the chest to build a foundation for the later chest lift.

Phase two: chest lift, arm lift and body contouring

The second phase focused on the upper body. When you lose this much weight, a visible scar is required to lift the chest, the same principle as a breast lift in women. Dr. Roos performed a full chest lift with nipple repositioning using an inferior pedicle technique, moving the nipple from its descended position to the border of the pectoral muscle.

At the same time, Dr. Emer contoured the calves using vaser and Renuvion, sculpted the arms with skin removal and closure in three layers, and grafted fat to the shoulders to create a broader, more athletic upper body. Morpheus radiofrequency was applied to the inner thighs to tighten loose skin in areas not being lifted surgically.

Phase three: thigh lift and final sculpting

By the third phase, the patient had continued losing weight on peptides and was back in the gym. His new body shape had taken form, and the final surgery addressed the legs. Dr. Emer performed a full thigh lift with liposuction and vaser to the inner and outer thighs, then used Renuvion plasma to tighten areas not being lifted.

Additional fat was grafted to the upper chest to enhance the lift from phase two and to the buttocks for increased size and shape. The thigh lift incisions were tailored during closure to assess tension and ensure the skin came together without excessive pull.

Why this is a multi phase plan

Each phase was planned around recovery, healing and the patient's continued weight loss and training. Skin removal, fat grafting, plasma tightening and contouring were staged so the body could adapt and so each result could inform the next.
