What Dr. Emer did · MAXORIS

Every procedure. In order.

  1. 01

    Chest border sculpting MAXORIS™

Lateral and lower borders of the chest sculpted with cannula working directly on the bone, removing minimal fat to create angled definition and tighten the chest contour.

“So now we're sculpting the chest, especially the borders of the chest. And you guys can see we're literally right on the bone.”

    Dr. Emer, in the film
  2. 02

    Six-pack abdominal sculpting MAXORIS™

Meticulous sculpting to chisel out the rectus abdominis definition through the thin fat layer, planned to reveal the six-pack he built in training.

“We're going to give him that six-pack that he's been looking for.”

    Dr. Emer, in the film
  3. 03

    Lateral side sculpting MAXORIS™

Fat removed from the sides to create a more angled, tapered look from chest to waist.

“We're going to mark the lateral borders of the chest as well as the side as our goal is to give a more angled look here and tighten everything through the chest.”

    Dr. Emer, in the film
  4. 04

    Arm definition sculpting MAXORIS™

The arms are sculpted in the same session so their definition matches the chest and torso, keeping the whole upper body in proportion.

  5. 05

    PureGraft fat filtration

Fibrous fat filtered through the PureGraft system to isolate the highest quality adipose tissue for grafting.

“Now some of the fat's going to be fibrous. So the pure graft is going to help us filter that and give him the best fat we can for his chest.”

    Dr. Emer, in the film
  6. 06

Fat grafting to chest and shoulders

Filtered fat placed along the upper borders of the chest and shoulders to lift and build the gladiator contour, using the minimal fat available.

“Here, there's very minimal fat. We're going to use whatever fat we have to lift up his chest and shoulders to give him that really gladiator muscular look.”

    Dr. Emer, in the film
  7. 07

    Gynecomastia gland excision

Glandular tissue excised from beneath the chest after sculpting, performed in conjunction with the nipple lift.

“So, now I'm doing the gencomastic excision along with the nipple lift.”

    Dr. Emer, in the film
  8. 08

    Nipple lift

Gentle lift of just over one centimeter to reposition the nipples higher and match the new chest contour.

“He needed just a gentle lift. So, we're lifting him like a centimeter, little over a centimeter higher.”

Why patients fly in

What Dr. Emer does differently.

Sculpting the chest borders, not just the chest

Dr. Emer maps and sculpts the lateral and lower borders of the chest to create an angled, defined shape, an area many surgeons skip when performing chest contouring.

“This is an area again a lot of doctors kind of skip when they do chest contouring. They do just the chest itself, but they don't do the border.”

Removing and adding fat in one mapped plan

Before the first incision, he identifies the lower border where fat is removed and the upper borders where filtered fat is placed back, sculpting and building as one strategy.

“So, we want to identify the lower border where we're going to do the sculpting, but we want to identify the upper borders where we're going to add fat.”

Meticulous work on lean, muscular patients

With very little fat to remove, the cannula works directly on the bone, requiring slow, precise passes to avoid over-sculpting or injury.

“He doesn't have much fat. So we're really sculpting meticulously”

PureGraft filtration for fibrous fat

When the harvested fat is fibrous, the PureGraft system filters it to isolate the best adipose tissue for grafting, ensuring graft survival and contour.

“Now some of the fat's going to be fibrous. So the pure graft is going to help us filter that and give him the best fat we can for his chest.”

Combining sculpting, grafting, excision and lift in one session

Chest borders sculpted, fat grafted to the upper chest and shoulders, gynecomastia tissue excised, and nipples lifted, all planned and performed together to minimize downtime and maximize definition.

    The starting point

He built the mass in the gym. This session carved the detail.

He arrived already very muscular and consistent with training. The goal was not to build mass, it was to chisel out and see the six-pack, sculpt the chest, remove fat from the sides, and define the arms.

What Dr. Emer decided to do

Planned around a training schedule: definition carved, fat grafted, and tissue lifted in one visit.

  3. 03The session

    In the room, in order

Dr. Emer marked the lateral borders of the chest and the sides to create a more angled look and tighten everything through the chest. He sculpted the chest borders, working right on the bone because there was very minimal fat. Some of the fat was fibrous, so the PureGraft system filtered it to give the best fat possible for the chest.

    • Mark the borders and plan the angles
    • Sculpt the chest and remove fat from the sides
    • Filter and graft the fat
    • Excise tissue and lift the nipple

    What changed

Along with the sculpting, Dr. Emer performed a gynecomastia excision and a nipple lift. This patient needed just a gentle lift, raised a little over a centimeter higher. The combination of fat removal, fat grafting, and tissue excision created the definition and lift that training alone could not achieve.

Chest, abs, and sides sculpted as one plan / Patient story

Already muscular. Ready for definition.

He built the mass in the gym. This session carved the detail.

He arrived already very muscular and consistent with training. The goal was not to build mass, it was to chisel out and see the six-pack, sculpt the chest, remove fat from the sides, and define the arms.

Along with the sculpting, Dr. Emer performed a gynecomastia excision and a nipple lift. This patient needed just a gentle lift, raised a little over a centimeter higher. The combination of fat removal, fat grafting, and tissue excision created the definition and lift that training alone could not achieve.

Already muscular, ready for definition

The patient in this video is not starting from scratch. He is already very muscular and consistent with training. The goal is not to build mass, it is to chisel out and see the six-pack, sculpt the chest, remove fat from the sides, and define the arms.

In most cases, men who come in have extra fat and Dr. Emer is sculpting around that fat. Here, there is very minimal fat. The plan is to use whatever fat is available to lift the chest and shoulders, for a muscular MAXORIS™ look.

What the treatment actually does

Dr. Emer marks the lateral borders of the chest and the sides to create a more angled look and tighten everything through the chest. He identifies the lower border where sculpting will happen and the upper borders where fat will be added.

This is an area many doctors skip when they do chest contouring: they do just the chest itself, but they do not do the border. Dr. Emer sculpts the chest, especially the borders, working right on the bone. Because the patient does not have much fat, the sculpting is meticulous.

Some of the fat is fibrous, so the PureGraft system filters it to give the best fat possible for the chest. Part of the reason for sculpting on the chest is to get rid of the fat at the bottom.

How a session is built

Four moves. One session.

Planned around a training schedule: definition carved, fat grafted, and tissue lifted in one visit.

  1. 01

Mark the borders and plan the angles

The chest, sides, and abs are marked so the sculpting creates a more angled, tightened look.

  2. 02

Sculpt the chest and remove fat from the sides

Fat is removed from the lower chest and sides, working right on the bone to create definition.

  3. 03

    Filter and graft the fat

The PureGraft system filters the fat, and the best fat is used to lift the chest and shoulders.

  4. 04

Excise tissue and lift the nipple

Gynecomastia tissue is excised and the nipple is lifted to match the new contour.

The treatment, explained

What is high-definition chest sculpting?

Also called chest sculpting with fat grafting, six-pack abdominal sculpting, gynecomastia excision, nipple lift, male chest contouring, PureGraft fat grafting, chest and ab definition, gladiator chest sculpting, MAXORIS™ chest sculpting.

What it is
High-definition chest sculpting with fat grafting is a body contouring procedure for muscular men who want to chisel out the chest, define the six-pack, and lift the chest and shoulders using their own fat. It combines liposuction to remove fat from the lower chest, sides and abdomen, fat grafting to the upper chest and shoulders, gynecomastia excision to remove glandular tissue, and a nipple lift to reposition the nipples higher. The goal is to create definition that reads as natural at rest and unmistakable in motion, revealing the physique built in training.
How it works
Dr. Emer marks the lateral and lower borders of the chest and the sides to plan where fat is removed and where it is placed back. He sculpts the chest borders with a cannula working directly on the bone, removing minimal fat to create an angled, tightened look. Fat is also removed from the sides and abdomen to chisel out the six-pack. The harvested fat is filtered through the PureGraft system to remove fibrous tissue and isolate the best adipose cells. The filtered fat is then grafted along the upper borders of the chest and shoulders to lift and build the gladiator contour. Gynecomastia tissue is excised from beneath the chest, and the nipples are lifted to match the new chest shape. The entire procedure is performed in one session under anesthesia.
Who it is for
This procedure is for men who are already very muscular and consistent with training, who want to see the definition they have built but cannot achieve through diet and exercise alone. Ideal candidates have minimal body fat, a strong chest and abdominal foundation, and realistic expectations about what sculpting and grafting can reveal. It is not a weight loss procedure and is not for men who do not train. Candidacy, goals, and whether this approach fits your build are determined during consultation.
Recovery
Recovery depends on the extent of sculpting, grafting, excision and lifting performed. Most patients experience swelling, bruising, and soreness for the first week. A compression garment is worn to support healing and contour. Light activity resumes within days, with full training resuming based on how the session goes and how the body heals. Dr. Emer provides a detailed recovery plan at consultation.
When results show
Initial definition is visible once swelling subsides, typically within two to four weeks. The grafted fat settles over three to six months, and the final contour matures as the chest and shoulders heal. Results are long-lasting when maintained with consistent training and stable weight. The chest, abs, and sides appear more angled, defined, and lifted, with a natural look at rest and clear definition in motion.

Asked plainly, answered plainly

What men ask before they book.

How can filler help define both the jawline and neck area when there is a weak chin and sagging neck?+

This page covers chest and body sculpting. Dr. Emer does offer male facial sculpting as a related service, and jawline and neck concerns are evaluated during consultation. Candidacy, placement, and technique for any facial filler work are determined at that time on an individual basis.

Is this procedure for someone who does not train?+

No. This is a finishing tool for men who are already very muscular and consistent with training, who want the chest, abs, and sides to match the effort they are putting in at the gym. It is not a weight loss procedure and does not replace diet and exercise.

What is the PureGraft system?+

PureGraft is a filtration system that removes fibrous tissue and blood from harvested fat, isolating the highest quality adipose cells for grafting. This improves graft survival and contour, especially when the fat available is minimal or fibrous.

How much fat is needed for grafting?+

Very little fat is needed. Dr. Emer uses whatever fat is available from the chest, sides, and abdomen, filters it through PureGraft, and places it along the upper borders of the chest and shoulders. Even lean patients can achieve the lift and definition they want.

What is the downtime?+

Downtime depends on the extent of sculpting, grafting, excision, and lifting. Most patients are back to light activity within days, with full training resuming based on how the session goes and how the body heals. A compression garment is worn to support healing.

Will the results look natural?+

Yes. The goal is definition that reads as natural at rest and unmistakable in motion. Dr. Emer sculpts the chest borders, chisels out the six-pack, and lifts the chest using your own fat, so the result matches your physique and training.

Do I need more than one session?+

Some patients achieve their goals in one session. Others build in stages, adding more definition or grafting over time. Candidacy, goals, and whether one session or multiple sessions fit your plan are covered during consultation.

What is the difference between this and regular liposuction?+

Regular liposuction removes fat. High-definition chest sculpting removes fat from the lower chest and sides, sculpts the borders right on the bone, grafts filtered fat to the upper chest and shoulders, excises gynecomastia tissue, and lifts the nipples, all in one session. It is a complete reshaping, not just fat removal.
