What Dr. Emer did · MAXORIS

Every procedure. In order.

  1. 01

Stage one body lift with circumferential skin excision MAXORIS™

Full body lift excising excess skin from abdomen, flanks, back and love handles. Belly button repositioned higher and flatter. Deep dermis closure with absorbable sutures for strength. 5 to 8 pounds of tissue removed.

“We are going to excise all of this. Lift the B, get rid of the excess skin of the love handle.”

    Dr. Emer, in the film
  2. 02

Tumescent liposuction and fat harvest MAXORIS™

Debulking liposuction to harvest fat from torso, chest, flanks and pubic area. Tumescent fluid with protective medication to reduce bruising and swelling. Fat purified with PureGraft filtration, enriched with stem cells and PRP.

“We're harvesting as much of it as we can to be able to use this to build up his chest. It's going into our pure graph filtration. It's going to help us add our stem cells and purify the fat so we get the most survival and the best look.”

    Dr. Emer, in the film
  3. 03

Fat grafting to buttock, chest and penis MAXORIS™

Purified fat grafted to upper buttock for contour and perkiness, into chest muscle above and below for lift and foundation, and to penis shaft and corona for girth enhancement and rejuvenation. PRP and PMMA filler injected into grafting sites to increase blood supply and fat survival.

“By doing PRP injections and adding filler, filler specifically that's more permanent that builds a blood supply like PMMA, we can get that fat to survive much more readily.”

    Dr. Emer, in the film
  4. 04

    Renuvion skin tightening MAXORIS™

Renuvion energy applied to mid, upper mid and lower back, sides and lower chest border to tighten skin not being excised. Targets areas needing firming without additional scars.

“This extra amount of tightening is going to do so much more for him.”

    Dr. Emer, in the film
  5. 05

Stage two chest lift with additional liposuction MAXORIS™

Skin and superficial dermis excised from chest and sides. Additional liposuction to upper abs, chest and sides for fine-tuning and fat harvest. Subdermal plexus preserved for blood supply. Pedicle kept attached to chest wall at 8 cm base width. Scarpa's fascia used as strength closure.

“We only remove the skin. We keep the dermis and you see bleeding nicely. There's a subdermal plexus there that's going to keep this alive.”

    Dr. Emer, in the film
  6. 06

    Blending injections MAXORIS™

PRP and filler injected weeks later to smooth mild asymmetries and indentations, creating even contour.

“Right now we're blending in where he has some mild asymmetries. You guys could see a little indentation through here.”

Why patients fly in

What Dr. Emer does differently.

Fat is harvested, purified with stem cells and PRP, then grafted to rebuild volume instead of discarded

Every ounce of viable fat removed during liposuction is filtered through PureGraft, enriched with the patient's own stem cells and platelet-rich plasma, and strategically grafted to the chest, buttock and penis to build structure and volume where the body needs it after extreme weight loss.

“We're harvesting as much of it as we can to be able to use this to build up his chest. It's going into our pure graph filtration. It's going to help us add our stem cells and purify the fat so we get the most survival and the best look.”

PRP and PMMA filler injected into fat grafting sites to increase blood supply and fat survival

Dr. Emer injects PRP and permanent PMMA filler directly into the areas where fat has been grafted. The filler builds a vascular network that feeds the grafted fat, dramatically increasing survival rates compared to fat grafting alone.

“By doing PRP injections and adding filler, filler specifically that's more permanent that builds a blood supply like PMMA, we can get that fat to survive much more readily.”

Two-stage approach allows the body to heal and the plan to refine between surgeries

Splitting the work into two procedures reduces surgical time, allows the first stage to settle before the second, and gives Dr. Emer the ability to fine-tune liposuction, fat grafting and skin excision based on how the tissue responds. The chest lift in stage two is performed on a foundation that has already healed.

Renuvion tightens areas not being excised for a smoother, tighter surface without additional scars

Energy-based tightening is applied to the back, sides and lower chest where skin needs firming but does not need to be cut. This creates a tighter, more sculpted surface across the entire torso without extending the incision lines.

“This extra amount of tightening is going to do so much more for him.”

Scarpa's fascia used as the strength layer for closure, not just skin sutures

Dr. Rojos closes the deep fascial layer with absorbable sutures before closing the skin, distributing tension across a stronger plane and reducing the risk of wound separation or widened scars.

“We're going to use that fascia as our strength closure. It's going to keep this together.”

    What changed

Weeks later, stage two lifted the chest. Dr. Emer removed the remaining skin from the chest and sides, performed additional liposuction to harvest more fat, and grafted that fat to even out the chest and build a foundation for the lift. The chest skin was then excised and the remaining tissue was lifted and closed. Renuvion was applied to tighten areas that were not being cut but still needed firming. PRP and PMMA filler were injected into the fat grafting sites to increase blood supply and help the grafted fat survive.

One day after the chest lift, he described the pain as low and the recovery as better than expected. Weeks later, he returned for blending injections to smooth mild asymmetries. He described the transformation as total: not just the physical change, but the confidence to look down at his own body and feel comfortable in it for the first time since puberty.

From 330 pounds to 225, then from loose skin to a new shape

The patient in this video lost over 100 pounds. The weight came off through discipline and consistency, but the skin stayed behind: hanging from the torso, the chest, the flanks and the back. He came to Dr. Emer not to lose more weight, but to remove what the weight loss left and rebuild the shape underneath.

This was a two-stage surgical plan. Stage one removed the excess skin from the torso and back, contoured what remained, and harvested fat for grafting to the chest, buttock and male anatomy. Stage two lifted the chest, removed the remaining skin, and refined the contour with additional fat grafting and regenerative injectables.

What the first surgery removed and rebuilt

Dr. Emer and Dr. Rojos performed a full body lift: the excess skin from the abdomen, flanks, back and love handles was excised in one procedure. The tissue removed weighed between five and eight pounds. The belly button was repositioned higher and flatter, and the remaining skin was tightened with deep sutures and Renuvion energy to smooth and firm the surface.

Fat was harvested from the areas being debulked and purified using a filtration system with added stem cells and PRP. That fat was then grafted to the upper buttock for shape and lift, to the chest for volume and foundation, and to the penis for girth enhancement and rejuvenation. The goal was not just removal, but reconstruction.

How a two-stage body lift is built

Four moves. Two surgeries.

Planned around what the patient has already lost and what the body needs to rebuild.

  1. 01

Stage one: remove the skin and harvest the fat

A full body lift excises excess skin from the abdomen, flanks, back and love handles. The belly button is repositioned. Fat is harvested from the areas being debulked, purified with stem cells and PRP, and grafted to the buttock, chest and male anatomy.

  2. 02

Tighten what remains with Renuvion and deep sutures

Renuvion energy is applied to the back, sides and lower chest to tighten skin that is not being excised but still needs firming. Deep sutures close the tissue in layers for strength and smoothness.

  3. 03

Stage two: lift the chest and refine the contour

Weeks later, the remaining skin is removed from the chest and sides. Additional fat is harvested, grafted to even out the chest, and blended with PRP and PMMA filler to increase blood supply and help the fat survive. The chest is then lifted and closed.

  4. 04

    Blend and refine after healing

Once the swelling subsides, blending injections smooth any mild asymmetries or indentations that remain.

The treatment, explained

What is body lift after weight loss?

Also called skin removal surgery, post bariatric body contouring, circumferential body lift, panniculectomy, abdominoplasty after massive weight loss, chest lift for men, male breast reduction after weight loss, fat grafting and transfer, body reconstruction.

What it is
Body lift surgery after extreme weight loss is a reconstructive procedure that removes excess skin from the abdomen, flanks, back and chest that will not tighten on its own after significant weight loss. It is not a weight loss procedure. It is performed on patients who have already lost the weight and are left with loose, hanging skin that causes discomfort, limits movement, or prevents them from seeing the results of their weight loss. The surgery excises the excess skin, repositions the belly button, and uses the patient's own fat to rebuild volume in areas that have deflated.
How it works
The procedure is typically performed in two stages. Stage one removes excess skin from the torso, flanks, back and love handles through a circumferential incision, repositions the belly button, and harvests fat through tumescent liposuction. The fat is purified with PureGraft filtration, enriched with stem cells and platelet-rich plasma, and grafted to the buttock, chest and male anatomy. Renuvion energy is applied to areas that need tightening but are not being excised. Deep sutures close the tissue in layers. Weeks later, stage two lifts the chest by excising remaining skin and performing additional liposuction and fat grafting. PRP and PMMA filler are injected into the grafting sites to increase blood supply and help the grafted fat survive. After healing, blending injections smooth any mild asymmetries.
Who it is for
Candidates are patients who have lost significant weight, typically 75 pounds or more, and have excess skin that will not tighten with exercise, diet or non-surgical treatments. They must be at a stable weight for at least six months, in good overall health, and realistic about the recovery and scarring involved. This is not appropriate for patients who are still losing weight or who expect the surgery to make them smaller. It is reconstruction for patients who have already done the work and need a surgeon to remove what the body cannot tighten on its own.
Recovery
Recovery depends on how much tissue is removed and how many areas are treated. Patients wear compression garments and have drainage tubes for the first one to two weeks. Discomfort is typically described as tightness rather than pain. Most patients return to light activity within two to three weeks and resume exercise after six to eight weeks. Swelling subsides over several months. The final result is visible once the tissue has fully settled, typically six to twelve months after the final stage. Recovery timeline and restrictions are determined at consultation based on the individual case.
When results show
Results are visible immediately after surgery, with continued improvement as swelling subsides and the tissue settles. The excess skin is permanently removed. Fat grafting results depend on how much of the grafted fat survives, which is influenced by blood supply, technique, and the patient's healing response. Dr. Emer uses purified fat enriched with stem cells and PRP, and injects PRP and PMMA filler into the grafting sites to increase survival rates. Blending injections performed weeks after surgery smooth any remaining asymmetries. The transformation is total: not just the physical change, but the confidence to look down at one's own body and feel comfortable in it.

Why Dr. Emer

Reconstruction is not cosmetic touch-up.

It is a surgical plan built around what the patient has already accomplished and what the body needs next.

Dr. Emer has spent two decades refining body contouring techniques that are now taught internationally. He personally performs every procedure in his practice, and his work is sought out by patients who need precision: athletes, performers, executives, and people who have lost extreme amounts of weight and need a surgeon who understands the difference between removing tissue and rebuilding a shape.

This is not about making someone smaller. It is about removing what will not tighten on its own, contouring what remains, and using the patient's own tissue to build volume and structure where it is needed.

EMER METHOD™×RECODEX™

Fat is harvested and purified, not discarded

The fat removed during liposuction is filtered, enriched with stem cells and PRP, and grafted to the chest, buttock and male anatomy to build volume and structure where the patient needs it.

Renuvion tightens skin that is not being cut

Energy-based tightening is applied to areas that need firming but do not need excision, creating a smoother, tighter surface without additional scars.

Two stages allow the body to heal and the plan to refine

Splitting the work into two surgeries reduces surgical time, allows the first stage to heal before the second, and gives Dr. Emer the ability to refine the plan based on how the tissue responds.

Blending injections smooth the final result

After the swelling subsides, PRP and filler are used to smooth any mild asymmetries or indentations that remain, creating an even, natural contour.

Asked plainly, answered plainly

What patients ask before they book.

Is this a weight loss procedure?+

No. Body lift surgery is reconstruction for patients who have already lost significant weight and are left with excess skin that will not tighten on its own. It removes the skin, not the weight. Candidates must be at a stable weight for at least six months before surgery.

How much skin is removed?+

It depends on how much excess skin remains after weight loss. The patient in this case had 5 to 8 pounds of tissue excised from the torso, flanks and back in stage one. The amount varies by patient and is determined during consultation and pre-operative planning.

Why two stages instead of one surgery?+

Splitting the work into two surgeries reduces surgical time, allows the first stage to heal before the second, and gives Dr. Emer the ability to fine-tune the plan based on how the tissue responds. The chest lift in stage two is performed on a foundation that has already healed, allowing for more precise contouring and fat grafting.

What is the recovery like?+

Recovery depends on how much tissue is removed and how many areas are treated. Patients wear compression garments and have drainage tubes for one to two weeks. Discomfort is typically described as tightness rather than pain. Most patients return to light activity within two to three weeks and resume exercise after six to eight weeks. The patient in this case described the pain as low and the recovery as better than expected.

Will the fat grafting survive?+

Fat survival depends on blood supply, technique, and how the tissue heals. Dr. Emer uses purified fat enriched with stem cells and PRP, and injects PRP and PMMA filler into the grafting sites to increase blood supply and help the fat survive. Survival rates are higher with this approach than with fat grafting alone, but some resorption is normal and expected.

Where are the scars?+

The body lift incision runs circumferentially around the torso, typically along the belt line where it can be hidden by underwear or swim trunks. The chest lift incision runs around the areola and may extend horizontally depending on how much skin is removed. Scars fade over time but are permanent. Dr. Rojos uses deep fascial closure and precise suturing techniques to minimize scar width.

Can I have a body lift if I had bariatric surgery?+

Yes. Many patients who have body lift surgery lost weight through bariatric procedures such as gastric bypass or sleeve gastrectomy. The requirement is that you are at a stable weight for at least six months and in good overall health. Your medical history, including bariatric surgery, is reviewed during consultation.

What is the difference between a body lift and a tummy tuck?+

A tummy tuck removes excess skin from the abdomen and tightens the abdominal muscles. A body lift removes excess skin from the abdomen, flanks, back and sometimes the chest in a circumferential pattern, addressing the entire torso. Body lift surgery is typically performed on patients who have lost extreme amounts of weight and have excess skin in multiple areas, not just the abdomen.
