What Dr. Emer did · RE:SKN

Every procedure. In order.

  1. 01

    Midface volumization

Alloclae placed in the midface on bone and in tissue using a larger cannula to rebuild foundation structure lost to HIV-related lipoatrophy.

“I already did a little of the midface, nasal labial folds, under eyes, and temple.”

    Dr. Emer, in the film
  2. 02

    Under-eye and temple volumization

Alloclae placed in the infraorbital hollows and temporal fossa to eliminate shadowing and restore lateral facial volume.

  3. 03

    Nasolabial fold correction

Deep placement of cadaveric fat in the nasolabial folds to lift and soften the fold from the foundation.

  4. 04

    Prejowl and jawline contouring

Alloclae placed in the prejowl sulcus and along the posterior jawline to create definition and lift the anterior face.

  5. 05

    Pre-auricular volumization

Posterior placement around the ear to vector the face backward and lift the anterior tissues.

  6. 06

    Brow and temple fusion blending

Small amount of Alloclae placed on the brow bone and temple fusion line to eliminate step-offs and create a continuous contour.

“This is where I'm going to blend the OG curve to the temple and brow. This is where you got to refine what you're doing.”

Why patients fly in

What Dr. Emer does differently.

He has treated HIV-positive patients with facial volumization for nearly 20 years, longer than almost anyone in the United States.

Fat grafting and cadaveric fat survive exceptionally well in this population, and Dr. Emer pioneered the pan facial approach now studied by other physicians.

He plans every area as one system, not isolated injections.

Posterior volumization lifts the anterior face, brow blends into temple, jawline builds backward to create forward lift.

He places cadaveric fat on bone and in tissue, deeper than synthetic filler can reach.

The material rebuilds the skeletal foundation and stimulates the body to produce its own fat, which is why the result is permanent.

“The cool thing about this is it can be injected on bone or in tissue. And because of the sculpture, it'll stimulate collagen production throughout his face and hopefully give him tightening as well.”

He uses a larger cannula for volumization, which requires understanding of anatomy and tissue planes.

The subcutaneous layer is chosen because fat builds fat, and the technique creates structure that synthetic filler cannot replicate.

  1. 01Before

    The starting point

HIV-related lipo atrophy left him with hollowed cheeks, deep folds, and lost structure throughout the face.

Sculptra had not worked well for him in the past, and synthetic filler would have been expensive and temporary. Dr. Emer chose cadaveric fat (Alloclae) placed throughout the face: midface, under eyes, temples, nasal labial folds, prejowl, jawline, and brow, all planned and placed to lift and contour as one system.

“So, we're doing pantacial volumization. He had a lot has lots of volume loss in his face and to give him like a face like lift like effect to give him some contour and definition.”DR. JASON EMER · FROM THE VIDEO

What Dr. Emer decided to do

Pan facial volumization means every area is treated as part of one plan, not isolated injections.

“To do that with all this fillers, it'd be very expensive. It's synthetic. It wouldn't last as long. So, here I'm doing this with a fat that's from Kadaavvers.”DR. JASON EMER · FROM THE VIDEO
  3. 03The session

    In the room, in order

The patient left swollen, which is expected: the numbing medicine and the volume from the material itself create an overdone look on day one. The main swelling resolves within one to three days, and the final result is visible at two weeks as the material settles and integrates.

    • Place cadaveric fat throughout the face
    • Build the jawline posteriorly

    What changed

“Very deep volume that was lost. Now replenished. And then when that settles in, it'll soften and look great. Everyone always looks overdone initially.”DR. JASON EMER · FROM THE VIDEO

Pan facial volumization for HIV-related lipo atrophy / Patient story

Deep volume loss. Rebuilt in one session.

The patient left swollen, which is expected: the numbing medicine and the volume from the material itself create an overdone look on day one. The main swelling resolves within one to three days, and the final result is visible at two weeks as the material settles and integrates.

When volume loss is medical, not cosmetic

The patient in this video has HIV-related lipo atrophy: deep volume loss in the face that changes structure, not just surface. Synthetic fillers would be expensive, temporary, and would not address the underlying tissue loss. Dr. Emer chose a different approach: cadaveric fat (Alloclae) placed throughout the face to rebuild the foundation and stimulate the patient's own fat production.

This is pan facial volumization, meaning the entire face is treated as one plan: midface, under eyes, temples, nasal labial folds, prejowl, jawline, and brow. The material is placed on bone and in tissue, and it integrates over weeks as swelling resolves and the body responds.

Why cadaveric fat works differently in HIV patients

Dr. Emer has treated HIV-positive patients for nearly 20 years, and the outcomes with fat grafting and cadaveric fat are consistently strong in this population. The material survives well, stimulates collagen production, and becomes permanent as the body builds its own tissue around it.

Sculptra, which works for many patients, did not function as well for this patient in the past. Alloclae gave him the volume and structure he needed in one session, with the option to refine later using micro droplet silicone or additional filler on the bone.

How a session is built

One session. The entire face.

Pan facial volumization means every area is treated as part of one plan, not isolated injections.

  1. 01

    Assess and plan

The face is assessed for volume loss on bone and in tissue, and the plan is drawn to rebuild structure and lift.

  2. 02

Place cadaveric fat throughout the face

Alloclae is placed in the midface, under eyes, temples, nasal labial folds, prejowl, jawline, and brow using a larger cannula.

  3. 03

    Build the jawline posteriorly

Volumizing the area around the ear lifts the face anteriorly, which is why the jawline is part of a facial lift plan.

  4. 04

    Blend and refine

The temple and brow are blended to eliminate step-offs, and the entire face is refined so both sides finish symmetric.

The treatment, explained

What is pan facial volumization?

Also called Alloclae, cadaveric fat grafting, HIV facial lipoatrophy treatment, deep facial volume restoration, permanent facial volumization.

What it is
Pan facial volumization with Alloclae is the placement of cadaveric fat throughout the face to restore deep structure in patients with HIV-related facial lipoatrophy. The material is placed on bone and in tissue to rebuild the foundation, lift sagging areas, and stimulate the body to produce its own permanent fat. Dr. Emer treats the midface, under-eyes, temples, nasolabial folds, prejowl, jawline, and brow as one system, not isolated injections.
How it works
Alloclae is placed using a larger cannula in the subcutaneous layer and on bone. The posterior areas (pre-auricular region, back of jaw) are volumized to vector the face backward and lift the anterior tissues forward. The brow and temple are blended to eliminate step-offs. Numbing medicine is mixed into the material, and the face is overdone by 20 to 30 percent to account for settling. The material integrates with surrounding tissue and stimulates collagen and fat production.
Who it is for
Pan facial volumization with cadaveric fat is for HIV-positive patients with facial lipoatrophy who have lost deep volume in multiple areas. It is also for patients in whom Sculptra has not worked well or who want a permanent result without the expense and maintenance of synthetic filler. Candidacy is determined in consultation after Dr. Emer reviews submitted photographs and medical history.
Recovery
Patients leave swollen from the numbing medicine and the volume of the material itself. The main swelling resolves within one to three days. The final result is visible at two weeks as the material settles and integrates. Bruising is minimal when the procedure is performed correctly. Recovery specifics are discussed in consultation.
When results show
The result is permanent. The cadaveric fat integrates with the patient's tissue and stimulates the body to produce its own fat. The face appears overdone immediately after the session, then settles into the planned contour as swelling resolves. Refinement with micro-droplet silicone or additional filler can be performed once the base is set, which is decided in consultation.

EMER METHOD™×RECODEX™

It survives well in HIV patients

Fat grafting and cadaveric fat consistently produce strong outcomes in this population, which is why Dr. Emer has used it for nearly 20 years.

It stimulates collagen and tissue

The material integrates with the surrounding tissue and stimulates the body to produce its own fat, which is why the result is permanent.

It can be placed on bone or in tissue

Unlike synthetic filler, which is limited to soft tissue, cadaveric fat can be placed directly on bone to rebuild structure.

It can be refined later

Once the base is set, micro droplet silicone or additional filler can be used to refine specific areas.

Asked plainly, answered plainly

What patients ask before they book.

Is pan facial volumization with Alloclae permanent?+

Yes. The cadaveric fat integrates with your tissue and stimulates your body to produce its own fat, which is why the result is permanent. Once the material settles, the structure it creates becomes part of your face.

Why does the face look so swollen immediately after the procedure?+

The numbing medicine mixed into the Alloclae and the volume from the material itself create an overdone look on day one. Dr. Emer places 20 to 30 percent more than the final goal to account for settling. The main swelling resolves within one to three days, and the final result is visible at two weeks.

Why is cadaveric fat better than synthetic filler for HIV-related facial lipoatrophy?+

Fat grafting and cadaveric fat survive exceptionally well in HIV-positive patients, while synthetic filler would be expensive, temporary, and unable to address the deep tissue and bone volume loss that lipoatrophy causes. Alloclae rebuilds the foundation, stimulates your own tissue, and becomes permanent.

Can the result be refined after the initial session?+

Yes. Once the base is set and the material has integrated, micro-droplet silicone or additional filler can be used to refine specific areas. Dr. Emer also mentioned that longer-term fillers like Bellafill or Radiesse can be placed on the bone of the temple, back of jaw, and prejowl for further structure. Refinement plans are decided in consultation.

Why does Dr. Emer volumize the back of the jaw when the goal is to lift the front of the face?+

Everything from the posterior area is vectored backward, which lifts the face forward. When you lose volume in the pre-auricular area (around the ear), the anterior face sags. Volumizing posteriorly creates a lift anteriorly, which is why the jawline and pre-auricular region are part of a facial lift plan.

How long has Dr. Emer been performing pan facial volumization for HIV patients?+

Dr. Emer has been treating HIV-positive patients with facial volumization for nearly 20 years. He pioneered approaches that are now studied by other physicians, and he personally performs every procedure in his practice.

What is the difference between Alloclae and Sculptra for facial volume?+

Sculptra is a synthetic biostimulator mixed with 9 cc of fluid per vial, which creates significant temporary swelling. Alloclae is cadaveric fat that can be placed on bone and in tissue, integrates permanently, and stimulates the body to produce its own fat. In HIV-positive patients, fat-based treatments like Alloclae survive better and produce more consistent outcomes than Sculptra.
