Years of discipline, and the skin had not caught up.

The patient in this video lost a significant amount of weight and then spent years in the gym building muscle. He arrived strong and consistent, but loose skin masked the definition he had earned. Previous procedures, including liposuction, a body lift, and fat grafting, left some areas uneven and others with significant scar tissue.

Dr. Emer designed a two-phase plan. Phase one focused on the torso and chest: Vaser ultrasound to break through scar tissue, contouring to expose the abs and chest border, and plasma heating to tighten the skin internally. Phase two addressed the arms, legs, buttock, and back with sculpting, skin removal, and fat grafting.

Both phases were built around his training schedule. The contouring, tightening, and grafting were placed to work with muscle as he continued lifting, not against it.

A body earned twice

The patient in this video lost a significant amount of weight and then spent years building muscle. He arrived disciplined and strong, but the skin and underlying tissue had not caught up to the work he was putting in. Loose skin masked the definition he had earned, and previous procedures left some areas uneven.

Dr. Emer designed a two-phase plan: phase one focused on the torso and chest with definition and tightening, and phase two addressed the arms, legs, buttock, and back with contouring, fat grafting, and skin removal. The goal was to give him the body shape that matched the effort, so when he returned to the gym, the result would continue improving.

Phase one: torso, chest, and definition

Phase one started with tumescent fluid placed under the skin to lift the tissue away from the muscle. Because the patient had previous liposuction and a body lift, there was significant scar tissue. Dr. Emer used Vaser ultrasound to break through the adhesions and allow contouring of the stomach, the edges of the chest, and the lower abdomen.

Fat was removed where possible and filtered for grafting. Plasma heating was applied internally to tighten the skin to the muscle, particularly along the border of the chest and upper abs. A small amount of fat grafting was placed in strategic areas to add definition and introduce stem cells that support long-term tissue quality.

The patient also received a combination of hyaluronic acid gel (Voluma) and polymethyl methacrylate (Bellafil) in the genital area to add girth and length, with fat grafting layered in to provide stem cells and improve the integration of the filler.

How the two phases work

    Phase one: torso and chest

    Vaser ultrasound breaks through scar tissue, contouring exposes the abs and chest border, and plasma heating tightens the skin to the muscle. Fat grafting adds definition and stem cells where needed.

    Phase two: arms and legs

    Superficial fat is removed to expose muscle contour, Renuvion tightens loose skin internally, and fat grafting is placed just under the skin to add definition.

    Skin removal: thighs, back, and buttock

    A thigh lift removes loose skin from the medial thigh and knee. A back lift and buttock roll lift remove excess skin from the lower back and lateral hip. Renuvion is applied to areas where skin removal is not planned.

    Fat grafting for symmetry and volume

    Fat is grafted to the buttock to add volume and correct previous uneven grafting. The result is a more masculine contour that works with continued training.

Why two phases instead of one?+

Two phases allow recovery between sessions and let Dr. Emer see how the first phase integrates before planning the second. It also reduces the total time under anesthesia.

How long is recovery between phases?+

Recovery depends on the extent of each phase, but most patients wait several weeks to months between sessions to allow tissue to heal and see how the result develops.

Can I train between phases?+

Yes. Training between phases helps the grafted fat and tightened skin mold into the contour you build. Dr. Emer will clear you to return to the gym based on how each phase goes.

What if I had previous procedures that did not work?+

Dr. Emer routinely corrects previous work. Vaser ultrasound breaks through scar tissue, and fat grafting can even out areas that were over- or under-treated.
