What Dr. Emer did · MAXORIS

Every procedure. In order.

  1. 01

    Glandular tissue removal MAXORIS™

Excision of hypertrophied male chest gland tissue, leaving a thin disk of tissue under the nipple areola complex to maintain blood flow, innervation and prevent a scalloped or hollowed appearance.

“We want to remove this glandular tissue to give it more of a flat athletic musculine look. But we don't want to remove it all the way. We leave a thin disk of tissue underneath the nipple areola complex and that maintains the blood flow and inervation for the nipples.”

    Dr. Emer, in the film
  2. 02

    Areola lift and repositioning MAXORIS™

Periareolar incision to remove stretched, pigmented skin, tighten the areola complex and reposition the nipple to the lateral border of the pectoralis muscle for an athletic contour.

“Then after that we want to tighten the areola complex the around it and position it into a more of a muscular athletic look which is usually at the lateral at the side border of the pectoralis muscle.”

    Dr. Emer, in the film
  3. 03

Liposuction of chest, sides and upper abdomen MAXORIS™

Liposuction of the chest, lateral chest walls and upper abdominal area to sculpt definition and remove residual fat, creating a trapezoid or square chest shape.

“lipos section of the chest, sides of chest, all the way around it and upper abs”

    Dr. Emer, in the film
  4. 04

Fat grafting or filler for muscle tone MAXORIS™

Optional fat grafting or filler (Sculptra) to enhance pectoral contour and create muscle definition.

“Lipo section fat fat grafting for the muscle tone with with or without alo clay or filler.”

Why patients fly in

What Dr. Emer does differently.

The lift is part of the plan, not an afterthought

Most surgeons remove the gland and stop. Dr. Emer performs a periareolar lift to tighten the stretched skin and reposition the nipple, preventing the indentation and sagging that persist even after gland removal.

“This is the one thing that most doctors don't do. They don't do the gland with the lift. You still will have an indentation. When you squeeze and you want to flex, you want to get really lean, it's going to hang a little bit and it'll have an indent.”

A thin disk of gland tissue stays to preserve sensation and shape

Dr. Emer leaves a controlled layer of glandular tissue under the nipple to maintain blood flow and innervation, preventing complete loss of sensation and avoiding a scalloped, hollowed look.

“We leave a thin disk of tissue underneath the nipple areola complex and that maintains the blood flow and inervation for the nipples. So you don't completely kill the sensation in the nipple.”

The chest is sculpted as a geometric shape, not just flattened

Dr. Emer sculpts the chest into a trapezoid or square, removing density loss and teardrop sagging that comes with age or weight loss, creating edges and definition that read as athletic.

“So, we need to create it more of a trapezoid or a square and that's going to allow the edges to get contoured.”

Optional fat grafting or filler adds muscle tone

Fat grafting or Sculptra can be added to enhance the pectoral contour and create visible muscle definition, a step most practices do not offer in gynecomastia correction.

“Lipo section fat fat grafting for the muscle tone with with or without alo clay or filler.”

All steps are performed together as one system

Gland removal, areola lift, liposuction and optional fat grafting are designed to work as one plan. Performing them separately or omitting the lift produces an incomplete result.

“You have to do this all together or you don't get the same result.”

    The starting point

Hypertrophied male chest tissue that creates puffiness and stretches the nipple, no matter how much you train.

Gynecomastia is the enlargement of male chest gland tissue. It can be genetic, hormonal, or environmental. It creates a soft, puffy look that training cannot fix, and it stretches the nipple and areola over time.

“So, first of all, you guys see he has a nipple rings sitting here. We're going to have to remove that for surgery for two main reasons. Number one, we want the area to be sterile.”DR. JASON EMER · FROM THE VIDEO

What Dr. Emer decided to do

Gland removal, areola lift, liposuction, and optional fat grafting or filler, all designed to finish as one athletic chest.

“Now the surgery is going to involve first removal of the glandular tissue. You see his glandular tissue. The male chest gland is hypertrophied. That's called gynecomastia.”DR. JASON EMER · FROM THE VIDEO

    In the room, in order

The gland sits under the nipple. Removing it is the first step, but it is not the only step. Without tightening the stretched skin and repositioning the nipple, the chest can still read as loose or undefined.

Gland removal, lipo, and nipple lift in one plan / What gynecomastia is

It is not fat. It is gland.

The gland sits under the nipple. Removing it is the first step, but it is not the only step. Without tightening the stretched skin and repositioning the nipple, the chest can still read as loose or undefined.

What gynecomastia actually is

Gynecomastia is the enlargement of male chest gland tissue. It can be genetic, hormonal, or environmental, and it creates puffiness that stretches the nipple and areola. The result is a chest that reads as soft rather than flat, no matter how much the patient trains.

The patient in this video has hypertrophied glandular tissue sitting under the nipple. That tissue is what Dr. Emer removes to give the chest a flat, athletic, masculine look.

Why the gland removal is only part of the plan

Removing the gland alone leaves the skin stretched and the nipple positioned too low or too wide. Dr. Emer corrects both in the same session: he removes the excess gland, then tightens the areola and repositions it to sit at the lateral border of the pectoral muscle, where it reads as athletic.

He leaves a thin disk of tissue under the nipple to maintain blood flow and sensation. That layer prevents the nipple from looking scalloped or hollowed, and it keeps the chest from reading as over-corrected.

How the MAXORIS chest plan is built

Four moves. One shape.

Gland removal, areola lift, liposuction, and optional fat grafting or filler, all designed to finish as one athletic chest.

  1. 01

    Remove the glandular tissue

The hypertrophied gland is removed, leaving a thin disk of tissue under the nipple to maintain blood flow and sensation.

  2. 02

    Tighten and reposition the areola

A small incision around the nipple removes stretched, pigmented skin and repositions the areola to the lateral border of the pectoral muscle.

  3. 03

Liposuction of the chest and sides

Liposuction sculpts the chest, sides, and upper abdomen to create definition and remove any remaining fat.

  4. 04

Optional fat grafting or filler for muscle tone

Fat grafting or filler can be added to enhance the pectoral contour and create a more athletic, defined look.

The treatment, explained

What is gynecomastia correction?

Also called male breast reduction, gynecomastia surgery, male chest sculpting, areola lift, nipple repositioning, chest liposuction, pectoral contouring, MAXORIS™ chest.

What it is
Gynecomastia correction is the surgical removal of enlarged male breast gland tissue, combined with areola tightening, chest liposuction and optional fat grafting or filler to create a flat, athletic chest. Gynecomastia is the hypertrophy of male chest gland tissue, which creates puffiness, stretches the nipple and areola, and does not respond to diet or exercise. It can be caused by genetics, hormones or environmental factors. Dr. Emer's MAXORIS™ chest plan removes the hypertrophied gland, tightens and repositions the areola, sculpts the chest and sides with liposuction, and optionally adds fat grafting or filler for muscle definition. A thin disk of glandular tissue is left under the nipple to maintain blood flow, innervation and a natural contour. The areola is repositioned to the lateral border of the pectoralis muscle for an athletic look. The periareolar incision heals nearly invisibly at the edge of the areola.
How it works
The procedure is performed under general anesthesia or IV sedation. Dr. Emer makes a periareolar incision around the nipple and removes the hypertrophied glandular tissue, leaving a thin disk of tissue under the nipple to preserve sensation and prevent a hollowed appearance. Stretched, pigmented skin around the areola is removed and the nipple is repositioned to the lateral border of the pectoralis muscle. Liposuction is performed on the chest, lateral chest walls and upper abdomen to sculpt definition and remove residual fat. Optional fat grafting or filler (Sculptra) is added to enhance pectoral contour and create muscle tone. The incision is closed with sutures that sit at the natural border of the areola. The exact combination of steps is decided in consultation based on the degree of gynecomastia, tissue quality and the athletic result you are after.
Who it is for
Men with enlarged chest gland tissue (gynecomastia) who want a flat, athletic chest. Candidates include men with genetic, hormonal or environmentally caused gynecomastia, men with stretched or low-positioned nipples, men who have lost significant weight and have residual chest fullness or sagging, and men who want muscle definition and a sculpted chest contour. Candidacy is decided in consultation based on tissue quality, the degree of glandular enlargement, skin elasticity and your goals.
Recovery
Recovery time and restrictions are discussed in consultation and depend on the extent of the procedure. Most patients return to light activity within a few days and resume full exercise within a few weeks. Swelling and bruising are expected and resolve over the first few weeks. A compression garment is worn to support healing and contour. The periareolar scar heals nearly invisibly at the edge of the areola.
When results show
Results are visible immediately after the procedure, with continued improvement as swelling resolves over the first few weeks and months. The chest appears flat, athletic and defined, with the nipple positioned at the lateral border of the pectoralis muscle. The thin disk of glandular tissue left under the nipple maintains sensation and prevents a hollowed look. The periareolar scar fades and becomes nearly invisible over time. Final results are seen at several months as the tissue settles and the contour refines.

Asked plainly, answered plainly

What men ask before they book.

Will I lose sensation in the nipple after gynecomastia surgery?+

Dr. Emer leaves a thin disk of glandular tissue under the nipple to maintain blood flow and innervation. This preserves sensation and prevents the nipple from losing feeling. Complete removal of all glandular tissue can kill sensation and create a scalloped, hollowed look, which is why the thin layer is intentionally left.

What does the scar look like after the areola lift?+

The incision sits at the edge of the areola, where it heals as part of the natural border of the nipple. Once healed, the scar is nearly invisible. The periareolar approach allows Dr. Emer to remove stretched skin and reposition the nipple without a visible scar on the chest.

Is gynecomastia correction just gland removal?+

No. Dr. Emer's MAXORIS™ chest plan includes gland removal, areola tightening and repositioning, liposuction of the chest and sides, upper abdominal sculpting, and optional fat grafting or filler for muscle tone. Each step is designed to work with the others. Gland removal alone leaves stretched skin and a low or wide nipple, which still reads as undefined even after the gland is gone.

Why do I need the areola lift if the gland is removed?+

Removing the gland alone does not tighten the stretched skin or reposition the nipple. Without the lift, the chest can still indent when you flex or lean forward, and the nipple hangs slightly even after the gland is gone. The lift tightens the skin, repositions the nipple to the lateral border of the pectoralis muscle, and creates a flat, athletic contour that reads as trained.

Can fat grafting or filler really add muscle definition to the chest?+

Yes. Dr. Emer uses fat grafting or Sculptra to enhance the pectoral contour and create visible muscle tone. This is an optional step that adds definition and an athletic look. Fat grafting uses your own fat, harvested during liposuction. Sculptra is a biostimulatory filler that builds collagen over time.

How is candidacy for gynecomastia correction decided?+

Candidacy is decided in consultation based on the degree of gynecomastia, tissue quality, skin elasticity and the athletic result you are after. Dr. Emer evaluates the glandular tissue, the position and size of the areola, chest contour, and whether liposuction, fat grafting or filler will enhance the result. The exact combination of steps is tailored to your case.

What happens if I only remove the gland and skip the other steps?+

The chest will be flatter, but it will not read as athletic. The stretched skin and low nipple position remain, and the chest can still indent when you flex or lean forward. Without liposuction and optional fat grafting or filler, the chest lacks definition and muscle tone. Dr. Emer's approach is designed to finish as one shape, not a collection of separate corrections.

Does Dr. Emer perform gynecomastia correction in one session?+

Yes. Gland removal, areola lift, liposuction and optional fat grafting or filler are performed in one session. The steps are designed to work together, and performing them separately produces an incomplete result. The exact plan is decided in consultation.
